Federation of Organizations is looking for a full-time Licensed Social Worker for our Mixed-Use and Affordable Housing program in Far Rockaway, Queens. This individual will provide social services, support, and counseling to formerly homeless adults with severe mental illness and substance use disorders. The Social Worker will use their expertise to implement best practices to meet the resident’s physical, emotional, and social needs to ensure their ability to maintain housing stability while reporting and evaluating measurable outcomes.

QUALIFICATIONS:

  • Current licensure at the Master’s level (LMSW or LMHC)
  • Experience working with individuals in recovery from mental illness (preferred)
  • A valid and clean NY State driver’s license
  • Fingerprinting, criminal record check
  • Approval from the Justice Center and NYS Office of Mental Health

CORE & ESSENTIAL RESPONSIBILITY:

  • Efficiently process paperwork for new residents in accordance with protocols
  • Complete intake assessments and maintain referral documents in our system (Foothold)
  • Plan, schedule, organize, direct, and oversee the provision of all clinical services
  • Attend staff meetings, case conferences, trainings, and management meetings when needed
  • Serve as a liaison with medical, mental health, and substance abuse providers to ensure the level of care that residents deserve.
  • Conduct assessments, formulate individualized service plans, and provide referrals and ongoing support to assist residents in developing skills and achieving personal goals associated with their well-being and housing stability.
  • Provide crisis intervention and leadership in resolving resident issues, including one-to-one clinical support and appropriate follow-up.
  • Abide by the agency’s Compliance Plan and False Claims Policy by preventing, detecting, and reporting any potential healthcare abuse and fraud. Report any potential non-compliance and comply with all other federal, state, and local regulations applicable to this job description.
  • Complete all necessary Individual Authorizations in order to exchange information as well as screenings during the intake process.
  • Utilize all agency-based IT applications pertaining to this role in a proficient manner, including LMS and CPI training, as outlined in the agency’s training schedule.
  • Remain current on changing developments in an individual’s field of expertise.
  • Attend bi-weekly supervision, with prepared agenda items

SALARY: $60,000 Annually
