Federation of Organizations is seeking a Full Time Service Coordinator (LPN) to work at the Mixed-Use Affordable Housing by Herkimer Gardens, Brooklyn.
REPORTS TO: Program Supervisor
DEPARTMENT: Mixed-Use Affordable Housing
HOURS: 35 hours per week
SALARY: 70K Annually
QUALIFICATIONS:
Licensed Practical Nurse, Experience working with people with disabilities and aging population a plus. Fingerprinting, criminal record check, approval from NYS Office of Mental Health
JOB FUNCTION:
Provide assistance to individuals age 55+, with chronic health conditions who need assistance with activities of daily living, to manage their medical conditions, maintain independent living and promote community inclusion. Provide support and education, coordination of care and interface with medical providers and community resources to assist individuals served.
