Federation of Organizations is looking for a full-time Service Coordinator (LPN) to work at the Mixed-Use Affordable Housing by Herkimer Gardens, Brooklyn.

QUALIFICATIONS:
Licensed Practical Nurse, Experience working with people with disabilities and aging population a plus. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

CORE & ESSENTIAL RESPONSIBILITY:
Provide assistance to individuals age 55+ with chronic health conditions who need assistance with activities of daily living to manage their medical conditions, maintain independent living and promote community inclusion. Provide support and education, coordination of care and interface with medical providers and community resources to assist individuals served.
SALARY: 70,000 Annually
