JOB TITLE:                         OutreachCare Coordinator

REPORTS TO:                     Program Supervisor/Manager

DEPARTMENT:                   Care Coordination

LOCATION:                          Remote- Long Island (Nassau and Suffolk)

CLASSIFICATION:               Non-Exempt

HOURS:                                35 Hours per week

QUALIFICATIONS:               

Minimum of a Bachelor’s degree or a CASAC with at least 2 years of experience serving people with mental illness, developmental disabilities or alcoholism or substance abuse.Clean, valid NYS Driver’s License. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

JOB FUNCTION:   

To ensure all enrollment targets for care coordination services are met within the DOH timeframe and as per agency guidelines.  To work within a team to develop effective outreach strategies and will continuously re-evaluate all activities to ensure successful enrollment.  The Coordinator will be assigned a monthly caseload, and will conduct outreach as per Department of Health protocol to work towards enrollment and will effectively maintain relationships with referral sources and interfacing agencies to ensure maximum enrollment.  The Coordinator will complete all appropriate assessments, including but not limited to HARP assessments. Work to successfully market care coordination services to potential participants in conjunction with various health homes and other outreach staff. Conduct presentations in the community and participate in community events.
