JOB TITLE:               HARP Assessor  

LOCATION:              Currently Remote (May require travel in the future throughout LI and NYC)

REPORTS TO:           Director

DEPARTMENT:        Care Coordination

CLASSIFICATION:  Non-Exempt

HOURS:                     35 Hours per week

QUALIFICATIONS:          

Bachelor’s Degree in human services plus 2 years’ experience. Clean, valid NYS Driver’s License. Fingerprinting, criminal record check, and all necessary clearances from the Justice Center.

JOB FUNCTION:   

To complete comprehensive NYS Eligibility Assessment of  individuals in health and recovery plans. The staff member will assess the individual’s behavioral health and social history and will provide eligibility for HCBS services.
