REPORTS TO:                                Program Supervisor- Aging in Place Housing Support (APHS)

DEPARTMENT:                              Residential Services

CLASSIFICATION:                         Non-Exempt

HOURS:                                        35 hours per week

SALARY: $33k per year

QUALIFICATIONS:                        

High School Diploma and at least 2 years experience in mental health.  BA in Human Services with no experience accepted. Clean valid NY State Driver’s License.  Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

JOB FUNCTION:

Provide case management services, supports and coordination of services to individuals 55 and over with psychiatric disabilities living in scattered apartment settings with the goal of keeping the individuals independently and stably housed.
