Federation of Organizations is seeking Full Time Care Coordinator with a flexible schedule & Remote work in the field however, employees have to report to the Medford or Queens offices for mandatory meetings.
DEPARTMENT: Care Coordination
HOURS: 35 Hours per week
Minimum of a Bachelor’s degree or a CASAC with at least 2 to 5 years of experience serving people with mental illness, developmental disabilities or alcoholism or substance abuse.Clean, valid NYS Driver’s License preferred. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
Provide care coordination and health promotion to individuals with severe mental illness and/or 2 or more chronic health conditions to avoid costly inpatient hospitalizations, emergency room visits and unnecessary care. Manage a caseload size of approximately 25 individuals and assist with other duties as needed. Oversees and coordinates access to all services (behavioral, medical health and social service supports) to assist individuals to stay healthy. Completes all required documentation according to standards and no later than 24 hours of service delivery.
