HOURS: 35 hours per week
QUALIFICATIONS:
Minimum of a Bachelor’s Degree and 2 years or a Master’s Degree and 1 year of relevant experience serving people with mental illness, developmental disabilities or alcoholism or substance abuse. Clean, valid NYS Driver’s License preferred. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
JOB FUNCTION:
Provide care coordination and health promotion to individuals throughout Long Island and NYC (Bronx, Queens, Brooklyn, and Manhattan). Provide Care Coordination to those with severe mental illness and/or 2 or more chronic health conditions to avoid costly inpatient hospitalizations, emergency room visits and unnecessary care. Assist individuals in obtaining Food Stamps and Social Security benefits. Oversees and coordinates access to all services (behavioral, medical health and social service supports) to assist individuals to stay healthy. Manage a specialized caseload of 12 individuals and assist with other duties as needed. Completes all required documentation according to standards and no later than 24 hours of service delivery.
