Peer Bridger Program


Peer bridgers work alongside the Suffolk County Bridger Program, delivering on-site comprehensive short-term case management services to individuals who are awaiting admission into treatment programs.  Because of their own personal journeys, peer bridgers are uniquely qualified to promote hope and encouragement to those negotiating their own recoveries. Peer bridgers assist individuals in becoming involved with self-help and recovery programs that help prevent relapse.  

Peer bridgers provide

•    Assistance in applying for benefits and entitlements
•    Transportation to appointments
•    Assistance in obtaining medication
•    Assistance in securing housing
•    Information and referrals to community resources
•    Peer support
