Case Management Queens
Federation of Organizations offers case management services to individuals in recovery in Long Island, Queens and Brooklyn. The Case Management Program helps people to live successfully in the communities of their choice by acquiring the necessary services to maintain a decent quality of life (such as housing, clinical treatment, entitlement benefits, vocational development and job placement). Through support and empowerment, case managers help people to set goals and create the plans to achieve them.
Federation recognizes the uniqueness of each individual and that everyone has the capacity for personal growth. Efforts are made to ensure that the care given to each person is individualized, culturally appropriate and non-discriminatory.
