Personalized Recovery Oriented Services (PROS)

Recovery Concepts is a comprehensive rehabilitation program that brings together rehabilitation, support and clinic services all under one roof. The program provides a range of services that can be personalized to assist an individual in meeting their personal goals. The services are provided individually or in a classroom setting, and are based on evidence-based practices, which means they are proven to work!

An Individual Recovery Plan (IRP) outlines a course of care and is built on an individual’s personal ambitions and hopes. The plan keeps everyone working in the same direction, which is toward his/her personal goals. The development of an IRP will be done in partnership with staff and can include anyone the individual feels will help in supporting their recovery.

Services include rehabilitation, support and clinic treatment. The individual can pick and choose which is right for them.

• Community Rehabilitation and Support (CRS)
These services are designed to help develop the skills and supports necessary for each individual to achieve wellness. Some examples of these services are designing a wellness plan to help in recovery; helping each person to determine what type of employment is right for them; budgeting and financial management; building relationships; and learning how to manage their mental health while avoiding the use of substances.

• Ongoing Rehabilitation and Support (ORS)
ORS services are designed to assist in maintaining employment. In order to qualify for this service, the individual needs to work a minimum of 15 hours per week. Examples of this service are vocational counseling, advocacy and support while on the job. These services can take place at the work place or outside of work hours, depending on what works best for each individual.

• Intensive Rehabilitation (IR)
These services are designed to help attain specific life roles, such as returning to school, obtaining employment, securing volunteer work or finding a new place to live. They may also include staying clean and sober, maintaining good mental health or meeting with loved ones to provide them with education and support. Once a specific goal is established, our knowledgeable and experienced professionals will assist the individual in achieving the goal.

• Mental Health Treatment
These services help each person to manage their illness so that they can pursue their personal goals. They include clinical counseling, medication management, health assessment, symptom management and treatment for substance abuse.

In order to qualify, you must be at least 18 years of age or older and have a history of mental illness and be a resident of Suffolk County.

We accept Medicaid, Medicare and self-pay. Transportation needs are addressed. Lunch is available.

The Foster Grandparent Program Goes Back to School!
