ACT Team Specialist

West Babylon- ACT Team

Lead responsibility for integrating peer and self-help information and services with the tasks of all team members, as a member of a mobile, multi-disciplinary treatment team for persons with severe and persistent mental illness.

Qualifications: Completion of a peer advocacy training program. At least 1 year of experience working or volunteering in a treatment or rehabilitation setting.  Fingerprinting, criminal record check, approval from New York State Office of Mental Health.

 

Care Coordinators

Nassau, Queens & Suffolk Counties

Provide care coordination and health promotion to individuals with severe mental illness and/or have 2 or more chronic health conditions. This will include linkage to behavioral, medical, residential, social, financial, and vocational resources; collateral contacts with both behavioral and medical treatment, follow up with referral and treatment appointments, and education of health promotion and preventive care. The care manager will communicate with the treating clinicians to discuss enrollee’s care needs, conflicting treatments, change in condition, etc. which may necessitate treatment change (i.e., written orders and/or prescriptions).

Qualifications: 1 - 2 yrs. experience with people with mental illness or people with medical conditions or a Bachelor’s Degree. Spanish speaking or multi-lingual a plus. Clean, valid New York State Driver's License.  Fingerprinting, criminal record check, approval from New York State Office of Mental Health.

 

Case Managers

Supported Housing- Suffolk County Region- Brentwood

Specialized Supported Housing- Queens region- 6th Street Residence

Specialized Supported Housing- Nassau Suffolk Region- Brentwood

Provide case management services, housing supports and coordination of services to people with psychiatric disabilities

living in scattered apartment settings.

Qualifications:  High School Diploma and at least 3 years experience in mental health.  BA in Human Services and 1 year experience preferred. Clean valid New York State Driver’s License.  Fingerprinting, criminal record check, approval from                                      New York State Office of Mental Health.

 

Clinical Social Worker

Patchogue- PROS

Provide goal oriented verbal counseling or therapy including individual, group and family for the purpose of addressing the emotional, cognitive and behavioral symptoms of mental health and co-occurring mental health and substance abuse disorders. Participate as a member of a multidisciplinary treatment team.  Comply with program regulations and complete required documentation. 

Qualifications: LCSW.  Experience conducting group and individual therapy.  Valid and clean New York State Driver's License.  Fingerprinting, criminal record check, approval from New York State Office of Mental Health.

 

Clinical Social Worker- Fee for Service- Bi-lingual

Wyandanch- Wyandanch Clinic

Provide direct clinical care to clients, both adults and children, of the Wyandanch Clinic as a member of a multi-disciplinary       treatment team.

Qualifications: Bi-lingual. LCSW, LMSW or LMHC credential required. At least 1 year of training and/or experience in a clinical setting.  Experience conducting group and individual therapy. Experience providing family psycho-education and/or other family support services.  Experience working in an interdisciplinary team preferred. Fingerprinting, criminal record check, approval from New York State Office of Mental Health.

 

Counselor

Glen Cove CR

Provide supportive counseling, restorative services, ADL Training, and crisis intervention to people with psychiatric disabilities in a licensed residential program.

Qualifications: High School Diploma and at least 1 year of experience working in a mental health setting. BA in Human Services and some experience with persons with psychiatric disabilities preferred.  Clean valid New York State Driver’s License. Fingerprinting, criminal record check, approval from New York State Office of Mental Health.

 

Peer Counselor- Part-time

Glen Cove

Provide a positive role model of community functioning, informal counseling and peer supports to others with psychiatric disabilities currently living in residential programs.

Qualifications: Some experience living in a community residence program for people with psychiatric disabilities or in independent living.  Good mastery of basic ADL skills, Ability and desire to assist others in mastery of independent living skills. An understanding of peer support self-help philosophy. Clean, valid New York State Driver’s License.  Fingerprinting, criminal record check, approval from New York State Office of Mental Health.

 

Peer Specialists- Full-time

Supported Housing-Suffolk County Region- Brentwood

Ozone Park

Provide psychiatric and medical assessment and treatment as a member of a mobile, multi-disciplinary treatment team and in a clinic setting for persons with severe and persistent mental illness.

Qualifications: Currently licensed as a physician by the New York State Education Department and certified by, or be eligible to be certified by, the American Board of Psychiatry and Neurology. Valid and clean New York State Driver's License.  Fingerprinting, criminal record check, approval from New York State Office of Mental Health.

 

Recreational Specialist- 30 hours per week

Brentwood- IBR

Provide a positive role model of community functioning, informal counseling and peer supports to others with psychiatric disabilities currently living in residential programs.

Brentwood & Queens Village (IBR, SMR, 6th Street)

Provide supportive counseling, restorative services, ADL Training, and crisis intervention to people with psychiatric disabilities in a licensed residential program. Evenings and weekends a must.

Qualifications: BA in Human Services OR two years experience working with persons with psychiatric disabilities. Clean, valid New York State Driver’s License required.  Fingerprinting, criminal record check, and approval from New York State Office of Mental Health.
