Advocate- Part-time
Patchogue- Advocacy
Responsible for representing Federation and promoting Federation programs through visiting hospitals and community based programs, offering intensive peer support, conducting self help groups and advocating for consumers of mental health services.
Qualifications: High School Diploma or equivalent. Personal experience in Mental Health. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

Assistant Case Manager- part time
Suffolk County
Assist Case Managers by providing selected services to SCM/ICM clients.
Qualifications: High School Diploma or Equivalent, NY State Driver’s License. Registered, inspected, and insured vehicle, and personal experience with mental health system.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Care Coordinator(Bi-Lingual)
Ozone Park- Care Management
Provide care coordination and health promotion to individuals with severe mental illness and/or have 2 or more chronic health conditions. This will include linkage to behavioral, medical, residential, social, financial, and vocational resources; collateral contacts with both behavioral and medical treatment, follow up with referral and treatment appointments, and education of health promotion and preventive care. The care manager will communicate with the treating clinicians to discuss enrollee’s care needs, conflicting treatments, change in condition, etc. which may necessitate treatment change (i.e., written orders and/or prescriptions).
Qualifications: 1 - 2 yrs. experience with people with mental illness or people with medical conditions or a Bachelor or Master’s Degree. Spanish/English speaking. Clean, valid NY State Driver's License preferred.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Case Coordinator
West Babylon/Ozone Park- ACT Teams
Act as a member of two mobile, multi-disciplinary treatment teams who integrate information and expertise on dual-recovery treatment. Position is split between two locations (West Babylon and Ozone Park).
Qualifications: At least 1 year of training and/or experience in integrated mental health and substance abuse assessment and treatment.  Experience in coordination of care and services.  Valid and clean NY State Driver's License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Case Management Specialist- Part-time
Queens- QACC
Assist people in recovery from mental illness with accessing supports and services that will meet their individual needs.  To assist these individuals to achieve their maximum level of functioning and independence in the least restrictive environment of their choice.
Qualifications: Personal experience in the Mental Health System.  Bi-lingual preferred. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Community Health Worker
Remote- Medford- Keeping Families Healthy
Community Health Workers will primarily be working out in the community with specific target populations. CHW's will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes.
Qualifications: High School Diploma; Bachelor’s degree preferred. Fingerprinting, criminal record check. Clean, valid NY State Driver’s License and reliable transportation. Experience doing community health field work preferred. Bilingual English/Spanish preferred.

Community Rehabilitative Associate
Brentwood- TCR & IBR
Provide 1:1 services to residents while in the community. Assist in skill training to individuals with psychiatric disabilities. Aid in the integration to community living.
Qualifications: BA in Human Services with two years experience working with persons with psychiatric disabilities.  Fingerprinting, criminal record check. Approval from NY State Office of Mental Health. Clean, valid NY State Driver’s License.

Counselors
Glen Cove- Community Residence
Provide supportive counseling, restorative services, ADL Training, and crisis intervention to people with psychiatric disabilities in a licensed residential program.
Qualifications: High School Diploma and at least 1 year of experience working in a mental health setting. BA in Human Services and some experience with persons with psychiatric disabilities preferred.  Clean, valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Dual Recovery Specialist
Brentwood- TCR/CTI
Provide goal oriented individual and group counseling, crisis intervention, linkage to outside agencies for people with psychiatric disabilities and dual recovery issues while residing in a licensed residential program.
Qualifications: Master’s Degree in Human Services. License preferred. Knowledge of substance abuse treatment and other co-morbid diagnosis. Two years experience working  with persons with psychiatric disabilities. Clean, valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

EHR Specialist
Medford
Support Federation of Organizations, in conjunction with third party EHR vendors, Health Homes, and other IT department staff, in achieving business objectives by setting up, training, and providing ongoing support for all electronic health record systems. Additionally, assists in developing and supporting efficient use of all IT systems, as well as exploring how to best apply new and emerging technologies. 
Qualifications: Associates Degree and at least 2 years of experience working with an EHR system.  Knowledge of Health Homes and RHIOs strongly preferred.  Clean, valid NY State Driver’s License.

Health Office Rehabilitative Associate
Brentwood- IBR
Assist Program Supervisor and Program Manager in managing the daily operations of a licensed residential program providing transitional housing to people with psychiatric disabilities.
Qualifications: BA in Human Services plus 1 year experience working with persons with psychiatric disabilities OR 3 years experience working with persons with psychiatric disabilities.  Residential experience preferred.  Clean, valid NY State Driver’s License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

LPN
Brentwood- TCR
Provide management and administration of medications as a member of a multidisciplinary residential treatment team for persons with severe and persistent mental illness.
Qualifications: Currently licensed as LPN by the New York State Education Department. Valid and clean NY State Driver's License.  Minimum of 1 year of experience in a treatment or rehabilitation setting.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

LCSW
Patchogue- PROS
Provide goal oriented verbal counseling or therapy including individual, group and family for the purpose of addressing the emotional, cognitive and behavioral symptoms of mental health and co-occurring mental health and substance abuse disorders. Participate as a member of a multidisciplinary treatment team.  Comply with program regulations and complete required documentation. 
Qualifications: LCSW.  Experience conducting group and individual therapy.  Valid and clean NY State Driver's License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Peer Counselor
Glen Cove
Provide positive role model of community functioning, informal counseling and peer supports to others with psychiatric disabilities currently living in residential programs.
Qualifications: Some experience living in a community residence program for people with psychiatric disabilities or in independent living.  Good mastery of basic ADL skills, ability and desire to assist others in mastery of independent living skills. Program Manager
Home Based Support Services
Responsible for the overall operations of the Home Based Support Services Program including hiring of staff, training, supervision, evaluation of interdisciplinary team, coordination and implementation of services and activities, oversight of program documentation, reporting to funding sources, program evaluation and development, ensuring quality services and compliance with program regulations.
Qualifications: Master’s Degree in Health Care Related Services Field.  A valid and clean NY State Driver's License. Supervisory experience.  Experience in working in nursing home, hospital, or other  medical/institutional setting preferred.

Provide supportive counseling, restorative services, ADL training, and crisis intervention to people with psychiatric disabilities in a licensed residential program.

Rehabilitation Counselor- Part-time
West Babylon- PROS
Act as a guide or coach to help people in recovery from mental illness who are served by the PROS program to follow their own path to recovery.  Provide information, training and hands on assistance in skill building to individual members and groups. Provide peer support to members of the program.  Complete required documentation and comply with program regulations. 
Qualifications: BA or High School diploma with experience working in mental health.  A valid and clean NY State Driver's License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Rehabilitation Therapist- Part-time
West Babylon- PROS
Assist individuals served by the PROS program to overcome barriers associated with their mental health condition so they can achieve their personal goals. Provide information, training and hands on assistance in skill building to individual members and groups. Complete all required documentation and comply with program regulations.  Assist with outreach and conducting intakes.    
Qualifications: LMSW or Mental Health Counselor, LCSW preferred.  Experience working in mental health.  A valid and clean NY State Driver's License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.    

Relief Rehabilitative Associates- Per Diem
Brentwood & Queens Village (IBR, TCR, 6th Street)
Provide supportive counseling, restorative services, ADL Training, and crisis intervention to people with psychiatric disabilities in a licensed residential program. Evenings and weekends a must.
Qualifications: BA in Human Services OR two years experience working with persons with psychiatric disabilities. Clean, valid NY State Driver’s License required.   Fingerprinting, criminal record check, and approval from NY State Office of Mental Health.

2014 Highlights

FEDERATION OF ORGANIZATIONS HONORS SENIOR VOLUNTEERS
Federation of Organizations hosted a Senior Volunteer Recognition Luncheon on Friday, June 6th at the Melville Marriott. Over 250 guests attended the event, which honored senior volunteers from the Foster Grandparent and Senior Companion Programs. The celebration featured a full-course gala luncheon, music, and dancing.
