  Associate Director, Clinical Services- ACT Teams

Responsible for the oversight of all ACT teams within the Clinical Services Department. Responsible for ensuring effective and efficient operation. Tasks include but are not limited to: program evaluation and development, reporting, staffing, ensuring quality services and compliance with all guidelines, regulations and policies.

Clinical Social Worker- Fee for Service

Wyandanch- Clinic

Provide direct clinical care to clients as needed as a member of a multi-disciplinary treatment.

Qualifications: LMSW credential. LCSW preferred. At least 1 year of training and/or experience in a clinical setting.  Bi-lingual/Spanish speaking. Experience conducting group and individual therapy. FACT Program Assistant

Manage all documentation and, in conjunction with the team leader, administrative aspects of the team.

FACT Criminal Justice Liaison

Act as a member of a mobile, multi-disciplinary treatment team who integrates information and expertise regarding criminal justice policy and procedures in regard to individuals receiving services from FACT.

FACT Family Specialist

Integrate information and expertise in family psychoeducation and other family support services as a member of a mobile, multi-disciplinary treatment team for persons with severe mental illness and forensic involvement.

Qualifications: LMSW (LCSW preferred). At least one year of training and/or experience in family psychoeducation and/or other family support services. Valid and clean NY State Driver's License. Fingerprinting, criminal record check, approval from NY State

FACT Housing Specialist

Act as a member of a mobile, multi-disciplinary treatment team who integrates information and expertise on forensic housing services for individuals with severe mental illness and forensic involvement.

Qualifications: LMSW/LCSW preferred. An understanding of and experience with housing individuals with forensic involvement.  Knowledge of residential services, substance abuse services, evidence based practices, and psychiatric rehabilitation practice.  Clean, valid NY State Driver’s License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

FACT LPN- Part-time (14 hours per week)

Provide management and administration of medications, in conjunction with the psychiatrist and registered nurse as a member of a mobile, multidisciplinary treatment team for persons with severe mental illness and forensic involvement.

Qualifications: Currently licensed as a Licensed Practical Nurse by the New York State Education Department. Valid and clean NY State Driver's License. Minimum of one year of psychiatric experience in a treatment or rehabilitation setting.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

FACT Psychiatrist

Provide psychiatric and medical assessment and treatment as a member of a mobile, multi-disciplinary treatment team and in a clinic setting for persons with severe mental illness and forensic involvement. Act as back-up for Medical Director for the entire agency         in his absence.

Qualifications: Currently licensed as a physician by the NY State Education Department and certified by, or be eligible to be certified by, the American Board of Psychiatry and Neurology. Valid and clean NY State Driver's License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

FACT RN

Provide management and administration of medications, in conjunction with the psychiatrist, as a member of a mobile, multidisciplinary treatment team for persons with severe mental illness and forensic involvement.

Qualifications: Currently licensed as a registered professional nurse by the New York State Education Department. Valid and clean NY State Driver's License.  Minimum of one year of experience in a treatment or rehabilitation setting.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

FACT Substance Abuse Specialist

Act as a member of a mobile, multi-disciplinary treatment team who integrates information and expertise on dual-recovery treatment for individuals with severe mental illness and forensic involvement.

Qualifications: LMSW/LCSW/CASAC.  At least one year of training and/or experience in integrated mental health, abuse assessment treatment and evidence based practices. Valid and clean NY State Driver's License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health. 

 

FACT Team Leader

Supervise and provide direct services on a mobile, multi-disciplinary treatment team for persons with severe mental illness and forensic involvement.

FACT Team Specialist

Lead responsibility for integrating peer and self-help information and services with the tasks of all team members as a member of a mobile, multi-disciplinary treatment team for persons with severe mental illness and forensic involvement.

Qualifications: Completion of a peer advocacy training program. At least one year of experience working or volunteering in a treatment or rehabilitation setting. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

FACT Therapist

Integrate information and expertise in mental health treatment and therapy as a member of a mobile, multi-disciplinary treatment team for persons with severe mental illness and forensic involvement.

Qualifications: LMSW. At least one year of training and/or experience in integrated mental health assessment, treatment and evidence-based practices. Valid and clean NY State Driver's License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

FACT Vocational Specialist

A member of a mobile, multi-disciplinary treatment team for persons with severe  mental illness and forensic involvement, who integrates information and expertise on supported employment, job development, and rehabilitation methods that assist the client with developing skills in his or her environments of choice.

Qualifications: LMSW/LCSW or CRC, and a minimum of 1 year of training and/or experience in job finding, employment counseling or vocational rehabilitation. Valid and clean NY State Driver's License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

Program Supervisor

West Babylon- PROS

Assist the program manager in providing oversight of all program operations in addition to conducting individual and group therapy      to clients. Responsible for completing and reviewing required program documentation and clinical records while providing staff        with direction.

Qualifications: LCSW preferred. LMSW applicants must be working toward LCSW. SIFI Certification preferred or must be obtained within the first year. Experience conducting group and individual therapy. Supervisory experience required. Rehabilitation Therapists

Assist individuals served by the PROS program to overcome barriers associated with their mental health condition so they can achieve their personal goals. Provide information, training and hands on assistance in skills building to individual members and groups. Complete all required documentation and comply with program regulations.  Assist with outreach and conducting intakes.

Community Health Worker

Brentwood- Nursing Home Diversion Supportive Housing Program

Provide a positive role model of community functioning, informal counseling and support to those with health conditions desiring to maintain their residential independence within the community.  

Qualifications: Some experience in working with individuals with health conditions that require assistance from support services.  Mastery of basic ADL skills with an ability and desire to assist others in the development of independent living skills. Adult Home Plus Care Coordinators

Remote- Serving Kings, Queens and Bronx Counties

Serve as the single dedicated care manager for individuals with serious mental illness transitioning from adult home settings to supported housing.  Provide intensive care management throughout the transition process, from in-reach, to assessment to transition and ongoing. Responsible and accountable for coordinating all aspects of care for individuals served. Serve as the single point of contact between the individual and various healthcare and community based providers, housing contractors, family members, adult home operators, guardians and other supports involved with the person.  Link clients to appropriate services and support in the community. Work to decrease avoidable hospitalizations and emergency department visits and promote the health and well-being as well as the successful transition to supported housing for those served.

Qualifications: 4 years of experience serving people with mental illness and a Bachelor’s Degree in a related field or Master’s Degree in a related field with two years of experience. Assistant Case Manager- Part-time

ACM Program

Assist Case Managers by providing selected services to SCM/ICM clients.

Care Coordinators

Medford- Care Coordination- Nassau & Suffolk Regions

Ozone Park- Care Cooridination- Queens, Brooklyn, Bronx, Manhattan & Suffolk Regions

Provide care coordination and health promotion to individuals with severe mental illness and/or have two or more chronic health conditions. This will include linkage to behavioral, medical, residential, social, financial, and vocational resources; collateral contacts with both behavioral and medical treatment, follow up with referral and treatment appointments, and education of health promotion and preventive care. The care manager will communicate with the treating clinicians to discuss enrollee’s care needs, conflicting treatments, change in condition, etc. which may necessitate treatment change (i.e., written orders and/or prescriptions).

Qualifications: Minimum of a Bachelor’s degree or a CASAC with at least 2 to 5 years of experience serving people with mental illness, developmental disabilities or alcoholism or substance abuse.  Clean, valid NY State Driver’s License preferred. Case Management Specialists- Full-time & Part-time

Ozone Park- Queens Adult Home

Assist people in recovery from mental illness with accessing supports and services that will meet their individual needs.  To assist these individuals to achieve their maximum level of functioning and independence in the least restrictive environment of their choice.

Outreach Counselor- Part-time

Patchogue- POWER

Work with people who are homeless and recovering from mental illness and drug/alcohol abuse by providing outreach referrals, support and advocacy. 12-18 Hours per week.

Qualifications: High School Diploma or equivalent.  Personal experience in Mental Health and a clean NY State Driver's License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

Program Supervisor- Care Coordination

Ozone Park- Adult Home Plus

Provide supervision, training, evaluation and support to Adult home plus care coordination staff, working closely with the program manager.   Provide oversight of documentation and ensure compliance with policies and procedures, regulations and guidelines. Conducts staff meetings. Provide adult home plus care coordination services to individuals in the program.  Provide crisis intervention as needed.

Qualifications: A Bachelor’s degree with 4 years’ experience (or a Master’s degree with 2  years’ experience) in any of the following: social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, creative arts therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing. Licensed Masters Social Worker (LMSW) with a minimum of 2 years’ experience preferred. All experience must  be either providing direct services to persons diagnosed with mental disabilities, developmental disabilities, alcoholism or substance abuse. Leadership abilities. Respite Workers

Medford

Provide Respite care to the dependent adult on behalf of the caregiver.  Interact with individuals on a one-to-one basis and provide support including advocacy, companionship, and daily living skills (i.e. light meal preparation, light housekeeping).
