Lead responsibility for integrating peer and self-help information and services with the tasks of all team members, as a member of a mobile, multi-disciplinary treatment team for persons with severe and persistent mental illness.

Qualifications: Completion of a peer advocacy training program. At least 1 year of experience working or volunteering in a treatment or rehabilitation setting.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

Administrative Assistant

Wyandanch Clinic

General Office Support, Administrative Assistant

Qualifications: High School Diploma. Previous administrative experience.

 

Clinical Social Worker- Fee for Service- Bi-lingual

Wyandanch- Wyandanch Clinic

Provide direct clinical care to clients both adults and children of the Wyandanch Clinic as a member of a multi-disciplinary

treatment team.

Qualifications: Bi-lingual. LCSW, LMSW or LMHC credential required. At least 1 year of training and/or experience in a clinical setting.  Experience conducting group and individual therapy. Experience providing family psycho-education and/or other family support services.  Experience working in an interdisciplinary team preferred. Employment Development Specialist

Patchogue- PROS

Work with individuals in the PROS Program who have expressed a desire to find employment through evidenced based employment services based on the Individual Placement and Support model (IPS).  Develop a network of employers willing to hire individuals in recovery from a mental health condition.  Provide ORS services in the community.  Maintain appropriate records according to program regulations. 

Qualifications: Bachelors degree, and or a minimum of one  year experience in job finding, employment, counseling or vocational rehabilitation. Valid and clean NY State Driver's License. Fingerprinting, criminal record check, approval from NY State Office of

Mental Health.

 

Psychiatrist- Per diem, PT, FT

Ozone Park

Provide psychiatric and medical assessment and treatment as a member of a mobile, multi-disciplinary treatment team and in a clinic setting for persons with severe and persistent mental illness.

Qualifications: Currently licensed as a physician by the NY State Education Department and certified by, or be eligible to be certified by, the American Board of Psychiatry and Neurology. Valid and clean NY State Driver's License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

Rehabilitation Therapists

West Babylon & Patchogue- PROS

Assist individuals served by the PROS program to overcome barriers associated with their mental health condition so they can achieve their personal goals. Provide information, training and hands on assistance in skills building to individual members and groups. Complete all required documentation and comply with program regulations.  Assist with outreach and conducting intakes.    

Qualifications: LMSW or Mental Health Counselor, LCSW preferred.  Experience working in mental health.  A valid and clean NY State Driver's License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Keeping Families Healthy

 

Community Health Worker

Remote

Community Health Workers will primarily be working out in the community with specific target populations. Community Health Worker's will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes.

Qualifications: High School Diploma; Bachelor’s degree preferred. Criminal Record Check. Clean valid NY State Driver’s License and reliable transportation. Experience doing community health field work preferred. Bilingual English/Spanish preferred.

 

Nursing Home Diversion Program

 

RN

Brentwood

Provide management and administration of medications in the field, as a member of a mobile, multidisciplinary treatment team for individuals served by the Nursing Home Diversion Supportive Housing Program to assist with overcoming barriers associated with their health conditions to live in scattered site apartments successfully. Provide information, training and hands on assistance in skills building to individuals in their homes. Complete all required documentation and comply with program regulations. Clients will be in scattered sites.

Qualifications:Currently licensed as a registered professional nurse by the New York State Education Department. Valid and clean NY State Driver's License.  Minimum of 1 year of experience in a treatment or rehabilitation setting. Willingness to travel and use own car. 

 

Outreach Services

 

Activities Assistant- Part-time

West Babylon- Drop-In Program

Monday and Tuesday 4pm - 8pm

Responsible for developing and implementing activities for the evening drop in program in conjunction with the Site Supervisor, volunteers and members.  Complete program documentation.

Qualifications: High School Diploma, personal experience in Mental Health. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

Assistant Case Manager- Part-time

ACM Program

Hours and location vary depending on program

Assist case managers by providing selected services to SCM/ICM clients.

Qualifications: High School Diploma or Equivalent. Clean, valid NY State Driver’s License. Registered, inspected, and insured vehicle, and personal experience with mental health system.  Fingerprinting, criminal record and license check, approval from NY State Office of Mental Health. 

 

Care Coordinators

Queens, Brooklyn, Bronx and Manhattan

Provide care coordination and health promotion to individuals with severe mental illness and/or have 2 or more chronic health conditions. This will include linkage to behavioral, medical, residential, social, financial, and vocational resources; collateral contacts with both behavioral and medical treatment; follow up with referral and treatment appointments; and education of health promotion and preventive care. The care manager will communicate with the treating clinicians to discuss enrollee’s care needs, conflicting treatments, change in condition, etc. which may necessitate treatment change (i.e., written orders and/or prescriptions).

Qualifications: 1 - 2 yrs. experience with people with mental illness or people with medical conditions or a Bachelor’s Degree. Spanish speaking or multi-lingual a plus. Clean, valid NY State Driver's License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

Employment Specialist

Patchogue- POWER

Provide vocational skill development, support, and job placement assistance to individuals who are homeless, in danger of becoming homeless, or who have a history of homelessness.

Qualifications: BA or equivalent experience in vocational rehabilitation, placement and job coaching.  Clean, valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.    

 

Field Supervisor

Patchogue- Senior Companion Program

Provide direct supervision and support to volunteers through weekly team meetings, community outreach, transportation and training.

Qualifications: Bachelors Degree or equivalent experience in mental health, Clean NY State Driver's License.  Fingerprinting, criminal record check, approval from NYS Office of Mental Health. National Service Criminal History check, approval within Senior Corps guidelines. National Sex Offender Public Registry Check.

 

Financial Manager

Medford- Financial Management

Conduct intake with potential clients of the Financial Management Program to determine eligibility.  Responsible for completion of client records and maintenance.  Complete referral documentation for Social Security.  Prepare monthly budget for client based on client’s needs. Monthly contact with client and/or case manger for monitoring recipient’s status.

Qualifications: Bachelors Degree or Equivalent, two years case management experience preferred.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

Intensive Care Coordinators

Remote- Queens and Brooklyn regions

Provide care coordination and health promotion to individuals with serious mental illness transitioning from adult home settings to more independent community settings.  Serve as the liaison between the individual and various healthcare and community based providers; oversee, coordinate and link individuals to appropriate medical, behavioral and social services; work to decrease avoidable hospitalizations and emergency department visits and promote the health and well being of those served. 

 Qualifications: A Bachelor’s degree in one of the fields listed below:

social work, psychology, nursing rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child & family studies, sociology, or speech and hearing

OR

NYS licensure and current registration as a Registered Nurse and a bachelor’s degree; OR a NY State teacher’s certificate for which a bachelor’s degree is required.

AND

Four years of experience either:

  • Providing direct services to people with Serious Mental Illness OR
  • Linking individuals with serious mental illness to a broad range of services essential to living successfully in a community setting.

(A Master’s degree in a listed related field may substitute for two year’s experience)

Site Supervisor- Part-time

West Babylon- Drop-In Program

Monday/Tuesday- 4pm-8pm

Responsible for the operation of the evening drop-in programs including, development of activities, supervision and training of staff, organize and completion of documentation, and crisis intervention. Assist in maintaining the cleanliness of the center.

Social  Work Assessor- Per Diem and Full Time

Remote- Ozone Park- Care Coordination

Complete comprehensive psychosocial assessments of residents in recovery from mental illness, living in impacted adult homes in NY City.  Assess the resident’s behavioral health and social history and provide recommendations to the plan of care. Communicate with the treating clinicians and care coordinator to obtain information needed to complete the assessments.

Qualifications: Social worker (LCSW or LMSW), with a minimum of 2 years experience working with people with mental illness.  Clean, valid NY State Driver's License preferred.  Fingerprinting, criminal record check, approval from NY State Office of

Mental Health.
