Case Manager Specialist
Brooklyn- BACC
Assist people in recovery from mental illness with accessing supports and services that will meet their individual needs. To assist these individuals to achieve their maximum level of functioning and independence in the least restrictive environment of their choice.
Qualifications: Personal experience in the Mental Health Systems. Bi-lingual preferred. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
Community Health Worker- Part- time
Remote- Keeping Families Healthy
Work out in the community with specific target populations. Work closely with medical providers, primary care teams and other agencies to improve patient care and outcomes. Flexible 15-30hrs/week. Community Health Worker and supervisor will create a tentative weekly schedule, hours will vary dependent on client case load and scheduled home visits.
Qualifications: High School diploma. Bachelors degree preferred. Criminal record background check. Clean, valid NYS Driver’s license and reliable transportation. Community Rehabilitative Associate
Brentwood- TCR
Provide skill training to individuals with psychiatric disabilities. Provide education and support advancement of the community's understanding of recovery and integration of individuals with mental illness.
Qualifications: BA in Human Services OR High School Diploma with two years experience working with persons with psychiatric disabilities. Fingerprinting, Criminal Record Check, Approval from NYS Office of Mental Health, Clean valid NY State Driver’s License.

Assist the Coordinator in the day-to-day operation of a convenience store.
Qualifications: Minimum of 1 year experience in retail or food service, High School Diploma or GED.
Counselors
Massapequa CR & ISA (Remote)
Provide supportive counseling, restorative services, ADL Training, and crisis intervention to people with psychiatric disabilities in a licensed residential program.
Qualifications: High School Diploma and at least 1 year of experience working in a mental health setting. Health Office Rehabilitative Associate
Queens Village- 6th Street Residence & Brentwood- IBR
Assist Program Supervisor and Program Manager in managing the daily operations of a licensed residential program providing transitional housing to people with psychiatric disabilities.
Qualifications: BA in Human Services plus 1 year experience working with persons with psychiatric disabilities OR 3 years experience working with persons with psychiatric disabilities. Residential experience preferred. Clean, valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
Peer Counselor
Glen Cove
Provide positive role model of community functioning, informal counseling and peer supports to others with psychiatric disabilities currently living in residential programs.
Qualifications: Some experience living in a community residence program for people with psychiatric disabilities or in independent living. Good mastery of basic ADL skills. Ability and desire to assist others in mastery of independent living skills. Understanding of peer support self-help philosophy. Clean valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
Rehabilitative Associates- FT, Overnight & Relief

Act as a guide or coach to help people in recovery from mental illness who are served by the PROS program to follow their own path to recovery. Provide information, training and hands on assistance in skill building to individual members and groups. Provide peer support to members of the program. Complete required documentation and comply with program regulations.
Qualifications: BA or High School diploma with experience working in mental health. Personal Experience in Mental Health required. A valid and clean NYS drivers License. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
Rehabilitation Therapist
West Babylon- PROS
Assist individuals served by the PROS program to overcome barriers associated with their mental health condition so they can achieve their personal goals. Provide information, training and hands on assistance in skill building to individual members and groups. Complete all required documentation and comply with program regulations. Assist with outreach and conducting intakes.
Qualifications: LMSW or Mental Health Counselor, LCSW preferred. Experience working in mental health. A valid and clean NYS drivers license. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
RN- Part-time
Brooklyn- Brooklyn Clinic
Provides management and administration of medications, in conjunction with the psychiatrist, as a member of a multidisciplinary treatment team for persons with severe and persistent mental illness
Qualifications: Currently licensed as a registered professional nurse by the New York State Education Department. Valid and clean NY State driver license. Minimum of 1 year of experience in a treatment or rehabilitation setting. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.
Substance Abuse Specialist
Patchogue- PROS
Assist members in the PROS Program to recover from both mental illness and substance abuse by providing dual-recovery treatment and education for both members and their families using evidenced- based practices. Provide information and expertise on dual recovery treatment to other members of the PROS staff. Comply with Program regulations and complete program documentation.
Qualifications: CASAC. At least 1 year of training and/or experience in integrated mental health and substance abuse assessment and treatment. Valid and clean NY State Drivers License. Finger printing, criminal record check, and approval from NYS Office of Mental Health
