Behavioral Specialist/Support Supervisor

Brentwood-IBR

Provide assessment, support and skill training to individuals with psychiatric disabilities in their pursuit in becoming active, valued members in the community.  Qualifications: MA in Human Services and two years experience working with persons with psychiatric disabilities. Clean, valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

Care Coordinators

Nassau, Queens & Suffolk Counties

Provide care coordination and health promotion to individuals with severe mental illness and/or have 2 or more chronic health conditions. This will include linkage to behavioral, medical, residential, social, financial, and vocational resources; collateral contacts with both behavioral and medical treatment, follow up with referral and treatment appointments, and education of health promotion and preventive care. The care manager will communicate with the treating clinicians to discuss enrollee’s care needs, conflicting treatments, change in condition, etc. which may necessitate treatment change (i.e., written orders and/or prescriptions). Qualifications: 1 - 2 yrs. experience with people with mental illness or people with medical conditions or a Bachelor’s Degree. Spanish speaking or multi-lingual a plus. Clean, valid NY State Driver's License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

Case Managers

Brentwood- TCR SHA Suffolk County

Provide case management services, housing supports and coordination of services to people with psychiatric disabilities living in scattered apartment settings. Qualifications:  High School Diploma and at least 3 years experience in mental health.  BA in Human Services and 1 year experience preferred. Clean, valid NY State Driver’s License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

Clinical Social Worker

Patchogue- PROS

Provide goal oriented verbal counseling or therapy including individual, group and family for the purpose of addressing the emotional, cognitive and behavioral symptoms of mental health and co-occurring mental health and substance abuse disorders. Participate as a member of a multidisciplinary treatment team.  Comply with program regulations and complete required documentation. Qualifications: LCSW.  Experience conducting group and individual therapy.  Valid and clean NY State Driver's License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

Clinical Social Worker

Wyandanch- Wyandanch Clinic

Provide direct clinical care to clients, both adults and children, of the Wyandanch Clinic as a member of a multi-disciplinary treatment team. Qualifications: LCSW, LMSW or LMHC credential required. At least 1 year of training and/or experience in a clinical setting.  Experience conducting group and individual therapy. Experience providing family psycho-education and/or other family support services.  Experience working in an interdisciplinary team preferred. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

Dual Recovery Specialist/Support Supervisor

Brentwood- IBR

Provide goal oriented individual and group counseling, crisis intervention, linkage to outside agencies for people with psychiatric disabilities and dual recovery issues while residing in a licensed residential program. Qualifications: Master’s Degree in Human Services. License preferred. Knowledge of substance abuse treatment and other co-morbid diagnosis. Two years experience working  with persons with psychiatric disabilities. Clean, valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

Outreach Care Coordinators

Nassau, Queens & Suffolk Counties

Provide outreach via telephone to individuals meeting the criteria for care coordination.  Work to engage participants over the phone and get them interested in the care coordination service.  Schedule appointments for care coordinator to conduct field visits and place follow up calls to ensure participants were satisfied with services.  Conduct follow up calls and internet searches to find individuals who cannot be located via phone numbers or addresses provided by the Department of Health.  Document all outreach efforts in an electronic health record. Contact medical providers as needed for the purposes of outreach and assist care coordinators in finding resources in the community that can assist those they are coming in contact with.   Qualifications: Minimum of a high school diploma.  Spanish speaking or multi-lingual a plus. Sales experience preferred.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

Peer Counselor - Part-time

Glen Cove

Provide positive role model of community functioning, informal counseling and peer supports to others with psychiatric disabilities currently living in residential programs. Qualifications: Some experience living in a community residence program for people with psychiatric disabilities or in independent living.  Good mastery of basic ADL skills, ability and desire to assist others in mastery of independent living skills. An understanding of peer support self-help philosophy. Clean, valid NY State Driver’s License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

Peer Specialist - Part-time

Brentwood-ISA

Provide a positive role model of community functioning, informal counseling and peer support to individuals with psychiatric disabilities currently living in an apartment treatment residential program. Qualifications: Personal or shared experience with mental illness and/or substance abuse issues.  Ability and desire to assist others in mastery of independent living skills and the personal recovery process . Understanding of peer support, self-help philosophy. Clean, valid driver’s license.  Fingerprinting, criminal record check.  Approval from NY State Office of Mental Health/Justice Center.

Ozone Park

Provide psychiatric and medical assessment and treatment as a member of a mobile, multi-disciplinary treatment team and in a clinic setting for persons with severe and persistent mental illness. Qualifications: Currently licensed as a physician by the NY State Education Department and certified by, or be eligible to be certified by, the American Board of Psychiatry and Neurology. Valid and clean NY State Driver's License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

 

Recreational Specialist - 30 hours per week

Provide positive role model of community functioning, informal counseling and peer supports to others with psychiatric disabilities currently living in residential programs. Rehabilitation Counselor

PROS- Patchogue

Act as a guide or coach to help people in recovery from mental illness who are served by the PROS program to follow their own path to recovery.  Provide information, training and hands on assistance in skill building to individual members and groups. Provide peer support to members of the program.  Complete required documentation and comply with program regulations. Qualifications: BA or High School diploma with experience working in mental health.  A valid and clean NY State Driver's License.  Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Qualifications: BA in Human Services OR two years experience working with persons with psychiatric disabilities. Clean, valid NY State Driver’s License required. Fingerprinting, criminal record check, and approval from NY State Office of Mental Health.

Transportation Coordinator

Assist the Transportation Manager in the overall operation and coordination of transportation for Suffolk County PROS and other CSS Programs. Qualifications: High School Diploma or equivalent. Computer Literacy. Experience working in an office setting. Clean, valid NY State Driver’s License. Experience in the transportation field preferred.
