ACT Team Leader
West Babylon- ACT
Supervise and provide direct services on a mobile, multi-disciplinary treatment team for persons with severe and persistent mental illness.
Qualifications: A master’s degree or higher in social work, psychology, rehabilitation counseling (if rehabilitation counselor, C.R.C. preferred), OR licensure/registration as registered nurse (RNs are encouraged to be masters level or nurse practitioner), or physician. A valid and clean NY State Driver's License. Minimum 3 years of experience in a treatment or rehabilitation setting.  Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

Advocate
Patchogue- Advocacy
Responsible for representing Federation with interfacing agencies meetings and events and advocates for the needs of people in recovery from mental illness within the mental health delivery system.  Provide peer support, advocacy, education and information to staff, volunteers and program members.
Qualifications: High School Diploma or equivalent. 4 years experience working in the mental health field. Personal experience in mental health. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

Assistant Case Manager
Suffolk County
Assist Case Managers by providing selected services to SCM/ICM clients.
Qualifications: High School Diploma or Equivalent, NY State Driver’s License. Registered, inspected, and insured vehicle, and personal experience with mental health system.  Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

Case Managers- Part-time and Full time
FT- Brentwood- Suffolk Supported Housing
PT- Queens Village- Bldg 74 Supported Housing

Provide case management services, housing supports and coordination of services to people with psychiatric disabilities living in scattered apartment settings who were previously residing in DOH Adult Home settings.
Qualifications: High School Diploma and at least 3 years experience in mental health.  BA in Human Services and 1 year experience preferred. Clean valid NY State Driver’s License.  Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

Case Management Specialists
Brooklyn- BACC
Assist people in recovery from mental illness with accessing supports and services that will meet their individual needs. Assist these individuals to achieve their maximum level of functioning and independence in the least restrictive environment of their choice.
Qualifications: Personal experience in the Mental Health System.  Bi-lingual preferred. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

Community Assistant – Part-time(17 hours/week)
Brentwood- IBR
Provide skill training to individuals with psychiatric disabilities.  Provide education and support advancement of the community's understanding of recovery and integration of individuals with mental illness. 
Qualifications: BA in Human Services OR High School Diploma with two years experience working with persons with psychiatric disabilities.  Fingerprinting, Criminal Record Check, Approval from NYS Office of Mental Health, Clean valid NY State Driver’s License

Counselor
Glen Cove
Provide supportive counseling, restorative services, ADL training, and crisis intervention to people with psychiatric disabilities in a licensed residential program.
Qualifications: High School Diploma and at least 1 year of experience working in a mental health setting. BA in Human Services and some experience with persons with psychiatric disabilities preferred.  Clean valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

Peer Counselor
Glen Cove & Massapequa
Provide a positive role model of community functioning, informal counseling and peer supports to others with psychiatric disabilities currently living in residential programs.
Qualifications: Some experience living in a community residence program for people with psychiatric disabilities or in independent living.  Good mastery of basic ADL skills. Ability and desire to assist others in mastery of independent living skills. Understanding of peer support self-help philosophy, Clean valid NY State Driver’s License.  Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

Peer Specialist- PT or FT
Queens Village- NYC Supported Housing
Provide a positive role model of community functioning, informal counseling and peer supports to others with psychiatric disabilities currently living in residential programs.
Qualifications: Personal experience with mental health and/or substance abuse issues.  Understanding of peer support self-help philosophy.  Clean, valid driver’s license. Fingerprinting / Approval from NYS Office of Mental Health.

Rehabilitative Associates- FT, Overnight & Relief & PT

FT & Overnight- TCR
PT- Queens Village (6th Street)- Saturdays & Sundays 12-10
Relief- Brentwood & Queens Village (IBR, TCR, 6th Street)

Provide supportive counseling, restorative services, ADL training, and crisis intervention to people with psychiatric disabilities in a licensed residential program. Evenings and weekends a must.
Qualifications: BA in Human Services OR two years experience working with persons with psychiatric disabilities. Clean valid NY State Driver’s License required.   Fingerprinting, criminal record check, and approval from NYS Office of Mental Health.

Relief Counselors
Glen Cove CR
Provide supportive counseling, restorative services, ADL training, and crisis intervention to people with psychiatric disabilities in a licensed residential program.
Qualifications: High School Diploma and at least 1 year of experience working in a mental health setting, B.A. in Human Services and some experience with persons with psychiatric disabilities preferred.  Clean valid NY State Driver’s License.  Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

Social Worker- Per Diem
Brooklyn- BACC Clinic
Provide direct clinical care to residents of the Brooklyn Manor Adult Home.  Participate as a member of a multi-disciplinary treatment team serving residents of the Brooklyn Manor Adult Home.
Qualifications: LCSW credential. At least 1 year of training and/or experience in a clinical setting.  Experience conducting group and individual therapy. Experience providing family psycho-education and/or other family support services.  Experience working in an interdisciplinary team. Valid and clean NY State Driver's License.  Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

Senior Volunteer Recognition Luncheon
A Celebration of Senior Volunteers

Federation of Organizations hosted a Senior Volunteer Recognition Luncheon on Friday, May 3rd at the Huntington Hilton. The event served to honor senior volunteers from the Foster Grandparent and Senior Companion Programs and was well attended by over 300 guests. The day was celebrated with a gala luncheon and dancing.

2012 Highlights
On October 5th, Federation of Organizations hosted a Networking and Business Event at Russo’s on the Bay. The event, sponsored by Liberty Management Group and Lincoln Computer Systems, provided a forum for over 200 business professionals, hospital representatives and not-for-profits to discuss the future of healthcare. The Keynote Speaker was Jason Helgerson, NY State Medicaid Director .

On Friday, June 15th, Federation of Organizations hosted a Senior Volunteer Recognition Luncheon at the Crest Hollow Country Club. The event, which was well attended with over 500 guests, served to honor senior volunteers from the Suffolk County Foster Grandparent and Senior Companion Programs as well as the Nassau County Retired and Senior Volunteer Program.


On April 20th Federation celebrated the opening of our new Queens Village Community Residence/Single Room Occupancy (CR/SRO) with a ribbon cutting ceremony and luncheon for over 150 guests. Pictured are Senator Frank Padavan and Barbara Faron, CEO performing the ribbon cutting.
