Advocate
Patchogue- Advocacy
Responsible for representing Federation with interfacing agency meetings and events and advocate for the needs of people in recovery from mental illness within the mental health delivery system. Provide peer support, advocacy, education and information to staff, volunteers and program members.
Qualifications: High School Diploma or equivalent. 4 years experience working in the mental health field. Personal experience in mental health. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Assistant Case Manager
Suffolk County
Assist Case Managers by providing selected services to SCM/ICM clients.
Qualifications: High School Diploma or Equivalent, NY State Driver’s License. Registered, inspected, and insured vehicle, and personal experience with mental health system. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Behavioral Specialist
Brentwood-IBR
Provide assessment, support and skill training to individuals with psychiatric disabilities in their pursuit in becoming active, valued members in the community.
Qualifications: MA in Human Services and two years experience working with persons with psychiatric disabilities. Clean valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Case Managers- Part-time and Full time
FT- Brentwood- Suffolk Supported Housing
Provide case management services, housing supports and coordination of services to people with psychiatric disabilities living in scattered apartment settings who were previously residing in DOH Adult Home settings.
Qualifications: High School Diploma and at least 3 years experience in mental health. BA in Human Services and 1 year experience preferred. Clean valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Case Management Specialist- Part-time
Queens- QACC
Assist people in recovery from mental illness with accessing supports and services that will meet their individual needs. To assist these individuals to achieve their maximum level of functioning and independence in the least restrictive environment of their choice.
Qualifications: Personal experience in the Mental Health System. Bi-lingual preferred. Fingerprinting, criminal record check, approval from NY State Office of Mental Health

Case Manager- Part-time

Queens Village- Supported Housing

Community Assistant – Part-time(17 hours/week)

Provide skill training to individuals with psychiatric disabilities. Provide education and support advancement of the community's understanding of recovery and integration of individuals with mental illness.
Qualifications: BA in Human Services OR High School Diploma with two years experience working with persons with psychiatric disabilities. Fingerprinting, criminal record check, approval from NY State Office of Mental Health. Clean valid NY State Driver’s License.

Community Health Worker
Remote- Medford- Keeping Families Healthy
Community Health Workers will primarily be working out in the community with specific target populations. CHW's will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes.
Qualifications: High School Diploma; Bachelor’s degree preferred. Fingerprinting, criminal record check. Clean valid NY State Driver’s License and reliable transportation. Experience doing community health field work preferred. Bilingual English/Spanish.

Community Rehabilitative Associate
Provide 1:1 services to residents while in the community. Assist in skill training to individuals with psychiatric disabilities. Aid in the integration to community living.
Qualifications: BA in Human Services with two years experience working with persons with psychiatric disabilities. Fingerprinting, Criminal Record Check, Approval from NY State Office of Mental Health. Clean valid NY State Driver’s License.

Cook- Part- time
Queens Village- 6th Street Residence
Prepare and serve a nutritional and well balanced dinner meal on a daily basis in a licensed residential program for people with psychiatric disabilities.
Qualifications: Food Service Certificate. Experience with commercial cooking. Fingerprinting, criminal record check. Approval from NY State Office of Mental Health. Clean valid NY State Driver’s License.

Dual Recovery Specialist
Brentwood- RTS
Provide goal oriented individual and group counseling, crisis intervention, linkage to outside agencies for people with psychiatric disabilities and dual recovery issues for residents residing in a scattered site supported housing program.
Qualifications: Master’s Degree in Human Services. License preferred. Knowledge of substance abuse treatment and other co-morbid diagnosis, two years experience working with persons with psychiatric disabilities. Clean, valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Family Specialist
West Babylon- ACT
Integrate information and expertise in family psychoeducation and other family support services as a member of a mobile, multi-disciplinary treatment team for persons with severe and persistent mental illness.
Qualifications: MSW (CSW preferred). At least 1 year of training and/or experience in family psychoeducation and/or other family support services. Valid and clean NY State Driver's License. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Field Supervisor

Patchogue- Senior/Junior Companion Program
Provide direct supervision and support to volunteers through weekly team meetings, community outreach, transportation and training.
Qualifications: Bachelors Degree or equivalent experience in mental health. Clean N.Y. State Driver's License. Fingerprinting, criminal record check, approval from NY State Office of Mental Health. National Service Criminal History check, approval within Senior Corps guidelines. National Sex Offender Public Registry Check.

Intake Coordinator
West Babylon- PROS
Responsible for screening and conducting intakes for individuals seeking admission to the PROS Program. Conducts assessments to determine the individual’s needs and preferences and develop recommendation for the person’s Individual Recovery Plan. Conduct new member orientation and coordinate with all referral sources and collaterals. Maintain appropriate records according to program regulations.
Qualifications: Licensed Master Social Worker. Experience working with individuals in recovery from mental illness. Valid and clean NY State Driver's License. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Peer Counselor
Glen Cove

Program Supervisor

Patchogue- POWER
Responsible for the overall operation of the P.O.W.E.R. Program including staff recruitment, training, evaluations, supervision, documentation, program development, reporting of units and coordination of services.
Qualifications: Bachelors Degree or equivalent experience in mental health, clean N.Y. State Driver's License, supervisory experience. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Program Supervisor
Brentwood- Suffolk County Supported Housing
Supervise staff to ensure the smooth operation of Supported Housing residential programs serving people with psychiatric disabilities. Assist the Manager in all aspects of program supervision, management and development. Responsible for the clinical and administrative supervision of program staff.
Qualifications: Master’s Degree in Human Services and a minimum of 2 years of supervisory experience and 5 years of experience in mental health. Knowledge of adult home settings, substance abuse and mental health residential services. Knowledge of psychiatric rehabilitation practice. Fingerprinting, criminal record check, approval from NY State Office of Mental Health. Clean and valid driver’s license.

Rehabilitaton Therapist
Patchogue- PROS
Assist individuals served by the PROS program to overcome barriers associated with their mental health condition so they can achieve their personal goals. Provide information, training and hands on assistance in skill building to individual members and groups. Complete all required documentation and comply with program regulations. Assist with outreach and conducting intakes.
Qualifications: LMSW or Mental Health Counselor, LCSW preferred. Experience working in mental health. A valid and clean NY State Driver's License. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Rehabilitative Associates- FT, Overnight & Relief & PT
FT & Overnight- TCR & RTS
PT- Queens Village (6th Street)- Saturdays & Sundays 12-10
Relief- Brentwood & Queens Village (IBR, TCR, 6th Street)

Relief Counselors

Glen Cove CR
Provide supportive counseling, restorative services, ADL training, and crisis intervention to people with psychiatric disabilities in a licensed residential program.
Qualifications: High School Diploma and at least 1 year of experience working in a mental health setting, B.A. in Human Services and some experience with persons with psychiatric disabilities preferred. Clean, valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Registered Nurse
Brentwood- RTS
Provide management and administration of medications as a member of a mobile, multidisciplinary treatment team for persons with severe and persistent mental illness.
Qualifications: Currently licensed as a registered professional nurse by the New York State Education Department. Valid and clean NY State Driver's License. Minimum of 1 year of experience in a treatment or rehabilitation setting. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

Substance Abuse Specialist
West Babylon- PROS

Assist members in the PROS Program to recover from both mental illness and substance abuse by providing dual-recovery treatment and education for both members and their families using evidenced- based practices. Provide information and expertise on dual recovery treatment to other members of the PROS staff. Comply with Program regulations and complete program documentation.
Qualifications: CASAC. At least 1 year of training and/or experience in integrated mental health and substance abuse assessment and treatment. Valid and clean NY State Driver's License. Finger printing, criminal record check, and approval from NY State Office of Mental Health.

Supervisor Evening Drop-In- Part-time

Patchogue/West Babylon- Drop-In Program
Responsible for the operation of the evening drop-in programs including, development of activities, supervision and training of staff, organize and completion of documentation, and crisis intervention. Assist in maintaining the cleanliness of the center
Qualifications: BA or equivalent experience in mental health. Supervisory experience preferred. Clean, valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NY State Office of Mental Health.

2014 Highlights

FEDERATION OF ORGANIZATIONS HONORS SENIOR VOLUNTEERS
Federation of Organizations hosted a Senior Volunteer Recognition Luncheon on Friday, June 6th at the Melville Marriott. Over 250 guests attended the event, which honored senior volunteers from the Foster Grandparent and Senior Companion Programs. The celebration featured a full-course gala luncheon, music, and dancing.
