New Onsite Case Management Program in Far Rockaway Adult Home
In 2004, Federation of
Organizations started the first onsite Supportive Case Management Program
at Brooklyn Adult Care Center. Due to it’s success, Federation was then
able to begin another onsite Supportive Case Management Program in January
of 2006 at Queens Adult Care Center. In July, Federation once again
expanded and has now opened it’s doors in two new Adult Homes: New Haven
Manor located in Far Rockaway and The Sanford Home for Adults located in
Flushing. Federation is now providing onsite case managem ent services to
the residents in both homes including assisting with entitlement benefits
and vocational development, linking clients to community integration, and
providing recreational activities. These case managers work to maintain a
rapport with the clients they serve while coordinating with both their
clinical and medical treatment teams.
Federation Case Managers pride themselves on the fact that the relationships they build with their clients are based on mutual respect and a desire to collaborate to achieve goals. These programs offer a unique opportunity to facilitate change. Case Managers ensure that individuals receive comprehensive, coordinated and individualized care, while recognizing the person’s unique emotional, social, cultural and spiritual dimensions. Federation of Organizations is an ever expanding agency that looks forward to opening more programs so that we may reach out and help as many people in need as we can.
